The Como Park Living at Home/Block Nurse Program is a non-profit, community-based organization supported by private donations, grants, and a combination of city, county and state funding.

The mission of the Como Park Living at Home/Block Nurse Program is to organize community volunteers, professionals and family members to provide care for their neighborhood seniors in ways that support choice, dignity and independence.


 

Como Park Living at Home/Block Nurse Program (LAH/BNP) brings neighborhood health professionals and volunteer caregivers together to provide quality, long term care for adults age 65 and older. The Living at Home/Block Nurse Program serves seniors and the entire Como community by enabling our elders to live in their own homes for as long as possible thereby delaying or avoiding placement in nursing homes.

Professional Home Health Care 

Como Park LAH/BNP, in cooperation with Wilder Home Health, coordinates registered public health nursing and home health/homemaking services.

Public Health Nurses provide assessment and on-going case management to those who require skilled care. 

Home Health Aide/Nursing Assistants provide personal care such as bathing and dressing, as well as meal preparation and light housekeeping.

We affirm the value of people contributing in some manner for the services they receive. Therefore, home health care charges are billed on a sliding fee scale. 

Volunteer Service Coordination

We are available to meet with seniors to discuss their needs. A care plan is designed for each senior that may include one or more of the following.

Volunteer Support: Arranging one on one support with trained volunteers, who can offer a variety of services and support (companionship, transportation, shopping, seasonal chores), etc.

Information and Referral: A phone call to Como Park LAH/BNP can be a good first step toward connecting with other community resources.

Health Education and Promotion: Como Park LAH/BNP sponsors educational workshops, publishes health education information, and provides activities dedicated to promoting good health.

Caregiver Support

We also provide supportive services and information to the formal and informal caregivers of seniors in our community.

Respite Services: Trained respite volunteers are carefully matched with families who need a temporary break from caregiving. (Overnight and 24 hour care is not available but referrals can be made)

Education and Support: Ongoing education on the specific needs/concerns of caregivers is provided. This support includes the development of a peer support group for caregivers.

Information and referral: Find out about resources available to you as a caregiver.

Service coordination (including volunteer support) and caregiver supportive services are provided free of charge.
